Assertive Case Managers
Assertive Case Managers and Advanced Nurse Practitioners help people with long term conditions who have been admitted to hospital at least three times in the past 12 months.
The team – which is part of the community nursing service – helps patients look after themselves so they can be supported as close to home as possible.
If a patient is admitted, they work closely with hospital and community staff to enable a safe and early discharge.
Assertive Case Managers and Advanced Nurse Practitioners are supporting the implementation of IV therapy in the community.
The team works with Heart of England Hospital and Good Hope Hospital.
How to use the service
Patients can be referred to the service if they are over 18, registered with a GP in NHS Birmingham East and North, and have had three emergency admissions in last 12 month period.
Patients will NOT be referred if they have had fewer than three emergency admissions in the last year, unless identified on UHE list. Admissions for emergency surgery are also excluded.

Response times
- Referrals can be made 24 hours, 7 days a week.
- All urgent referrals must be contacted and if necessary visited within 4 hours.
- All non urgent referrals must be contacted and if necessary visited within 24 hours.
