Community (District) Nursing
The District Nursing (DN) Service provides nursing care over 24 hours, 7 days a week, 365 days a year in partnership with BADGER (out of hours) service. They provide nursing care principally to people with long term health conditions, over the age of 16 years in their own homes, in clinics and residential care settings. They care for the terminally ill and those who are acutely ill and need intensive and technological care or who have specialist care needs. The service aims to optimise health and well being, through anticipation and coordination of health and social care provision.
The Assertive Case Management model of long-term condition management has been integrated within the District Nursing Service and the Assertive Case Managers (ACM’s also known as Community Matrons), are members of the District Nursing teams. The District Nursing Evening Service has been aligned to the day District Nursing Service structure to support continuity and the delivery of seamless care packages.
The District Nurse service is the largest of NHS BEN CHS, employing more than 260 members of staff. Staffs are based within teams and are aligned to the practice based commissioning localities.
The service is available to anyone registered with a GP in NHS Birmingham East and North. Find out if you are covered by NHS Birmingham East and North.
How to use the service
You can refer yourself, as can a carer or relative. A healthcare professional, such as a GP, can also refer you. If you have any difficulty in accessing the service, please contact your GP or practice nurse.
Where we provide the service
The community nursing team work with your family doctor in your local neighbourhood. They are some times based in a health centre with your GP, or can be on an alternative site within the neighbourhood.
When we provide the service
Community nursing is a 24 hour service.
Most nursing takes place during the day. A team of nurses working in partnership with your GP.
In the evening the Badger service covers all unplanned nursing care. Badger is a service contracted to provide out of hours medical and nursing services.
