Diabetes
Diabetes is a long-term (chronic) condition caused by too much glucose (sugar) in the blood. It is also sometimes known as diabetes mellitus.
Our Community Diabetes Team provides group education sessions for people around the area with newly-diagnosed Type 2 diabetes in English and Urdu. There is also an expert diabetes training programme for people with Type 2 diabetes.
We also offer dose adjustment for normal eating courses for people with Type 1 diabetes.
People with diabetes have a risk of developing retinopathy (damage to the retina at the back of the eyes). NHS Birmingham East and North offers a free, state-of-the-art eye screening service for patients with diabetes.
Who can use the service
You can use service if you have diabetes and you are not attending the hospital diabetes clinic, but need more help with your diabetes than your GP or practice nurse can give. You or your GP need to be covered by NHS Birmingham East and North.

Your GP or practice nurse will refer you to us. You can contact us direct if you are interested in the expert diabetes programme using the details below.
Where we provide the service
The Community Diabetes Team is based in the PAK Health Centre in Washwood Heath. Clinics are held in various health centres in the area. You may also be seen by a member of the team in your own GP practice.
When we provide the service
Between 8.30am and 4.30pm Monday to Friday, excluding bank holidays.

What if I can’t use the service?
