Acute Community-Based Care Service

Some patients with respiratory conditions such as chronic obstructive pulmonary disease (COPD), asthma or pneumonia, would be better
cared for at home than in hospital. This service will help make this happen.

This new scheme allows patients with respiratory conditions and with an NHS Birmingham East and North GP to go home and into the care of the Acute Community-Based Care (AC-BC) service.

The AC-BC service will deliver safe and effective care to make it possible for you to stay at home if your respiratory condition gets worse when you would usually call an ambulance or have to go into hospital.

How does the Acute Community-Based Care service work?

The AC-BC service consists of six experienced nurses who will assess your condition, plan and deliver care, including prescribing drugs. The team is based in the community so that they can travel to your own home or residential home.

A care bureau provides 24 hour telephone support, so that you can get any advice you may need, and provides a rapid response team to go to your home if required.

You may be seen for review by the doctor when your treatment is complete and you are ready for discharge. You will stay on the AC-BC for five days at the most.
