Finding a GP

Use the NHS Choices service search to find a GP close to where you live.

Registering with a GP

To register with a GP, contact a local practice and ask to be registered. If they accept you, they will let you know how to register. It will usually help if you have your medical card to hand.

If you do not have a medical card, fill in form GMS1, which should be available at the surgery.

Having problems registering?

If you have contacted more than five local GPs with no success, you can call the Birmingham Primary Care Shared Services Agency on 0845 111 1200, who can allocate you to a local GP. In order to be allocated, you will need to provide the Agency with the reasons the GPs have given you for not taking you on.
