Perry Tree Centre

The Perry Tree Centre is a short-stay rehabilitation centre with 32 single bedrooms in Perry Common.

The Centre provides short-term day rehabilitation programmes to help patients improve their quality of life and live more independently. Treatment encourages you to build up stamina and strength to increase mobility, reduce the risk of falling, and manage and improve any short-term memory issues.

You can contact us directly by calling the referral line on 0845 600 4550. The line is open from 8am to 10pm daily.

Opening times

The residential unit is open 24 hours a day 7 days a week (although admissions should take place before 8pm where possible).

How to use the service

  • You can ask your GP, district nurse, social worker or any other health or social care professional to refer you.
  • In Good Hope or Heartlands hospitals, talk to the intermediate care liaison nurses to discuss your needs and manage your transfer to Intermediate care if agreed.
  • In other hospitals, ask the ward staff or your consultant about a referral to intermediate care.
  • You can contact us directly by calling the referral line on 0845 600 4550. The line is open from 8am to 10pm daily.
