Practice to move in autumn 2011
In autumn 2011, the Hodge Hill Family Practice will move to a new health centre on the same road. Services will include extended access to General Practice, phlebotomy services, community nursing, pharmacy, Birmingham City Council Neighbourhood Office services and a range of clinical and non-clinical facilities that can be used by required services on a bookable basis.

The practice employs a multi-skilled team of health professionals including male and female General Practitioners (GPs), Advanced Nurse Practitioner, Practice Nurse and Health Care Assistant.

Registering at the practice
If you live within three miles of our practice and would like to register with us please complete one of the registration forms available from our reception. You will be registering with the practice rather than an individual GP.
Appointments
Please call the surgery to make an appointment to see a GP or nurse.
Please say immediately if your call is urgent and you will be seen the same day. You can expect to see a GP within one or two working days and you have the right to see the GP of your choice.
If you don’t need an appointment within two working days, you also have the option to book up to one month in advance if this is more convenient for you. Appointments with the practice nurse are available within one working day. She can treat you for a wide range of common conditions for which she can prescribe medication if necessary.
All patients need individual appointments so let us know if more than one person in the family needs to be seen. Also, please tell us if you want someone to accompany you during your examination
Telephone advice
If you do not need an appointment but need some advice, the doctor may be able to speak to you by phone between 11am and 12noon.
Home visits
If you are too ill to come to the surgery and need a doctor to visit you at home, please try to ring before 10.30am if possible. Please remember that home visits are primarily for those who are housebound or too ill to go out. Otherwise do try to come to the surgery.

This information is only available to those involved in your care and you should never be asked for personal medical information by anyone not involved in your care. You have a right to know what information we hold about you. If you would like see your records please speak to our practice manager.
Zero tolerance
We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way.
We take seriously any threatening, abusive or violent behavior against any of our staff or patients. If patients are violent or abusive, they will be warned to stop their behavior. If they persist, we may exercise our right to take action to have them removed, immediately if necessary.
Patients with particular needs
Our surgery is accessible to patients with disabilities. When booking appointments, please let reception staff know if English is not your first language and we can arrange interpretation and translation services in person or by phone.
Specialist and hospital care
If a GP or another member of our healthcare team believes you need hospital treatment or specialist care elsewhere, they will ask you where and when you would like to go.
Repeat prescriptions
If you take medication on a long-term basis, you can request your prescription two working days before you run out by posting your prescription in the prescription box. If you are unable to come into the practice please call between 11am to 12noon Monday to Friday and we can arrange a repeat prescription for you.

If you are unhappy with any part of our service, you can either submit a complaint direct to the practice:

- In person – ask to speak to the Practice Manager
- In writing to the Practice Manager
