Family Liaison Service

This service ensures that a complete package of support is planned and delivered to both patients and their carers. The service will enable patients and their families to make informed choices about their future care needs, and ensure equity of access to the full range of available services.

This is an enhanced service that will support you, your family, GP and primary care services in accessing the care package that you need to enable you to be cared for in your own home.

The scheme aims to prevent avoidable hospital admissions by ensuring that patients and their carers feel well supported at home and have access to all of the care and services that they need, when they need them.

How does the Family Liaison Co-ordinator Service work?
The service consists of a team of experienced nurses based in the community, so that they can visit you in your own home or residential home. A Care Bureau provides 24 hour telephone support, where you can speak directly to a nurse to get any advice you may need or provide a rapid response nurse to come out to your home if required. Your own GP will continue to provide medical advice in the usual way.

You will also have a personal Family Liaison Co-ordinator who has been assigned to work closely with your GP. Your Family Liaison Co-ordinator will identify your concerns and needs for current and future care, and details will be shared with your GP, District Nursing Team and any other services involved in your care.

The Family Liaison Co-ordinator will keep in regular contact with you, making planned visits convenient to you. You can contact your Family Liaison Co-ordinator via the Care Bureau number.
