Enhanced Supported Discharge

The Enhanced Supported Discharge service helps patients return home from hospital following accidents.

Patients on the trauma wards at Heartlands Hospital and Good Hope Hospital in Birmingham, and registered with a NHS Birmingham East and North GP, could go home earlier and into the care of an Enhanced Supported Discharge (ESD) team.

How does the ESD service work?

The ESD team consists of nurses, physiotherapists and an occupational therapist, who will work closely with your GP to provide you with the care and support you will need straight after your discharge from hospital.

The team works seven days per week from 8am to 8pm and is based in the community so that they can travel to your own home or residential home. A care bureau provides 24 hour telephone support, so that you can get any advice you may need, and provides a rapid response team to go to your home if required.

What about my home?

Your home environment, equipment and domestic support, transport needs and medications will be assessed as part of your care and rehabilitation plan.

What happens next?

Once you feel able to manage independently you will be discharged from the care of the team, usually within the first week after your discharge from hospital. If you need any further care, such as from a district nurse, the ESD team will arrange this for you.
