Healthcare closer to home
NHS Birmingham East and North has entered into partnership with Healthcare at Home Ltd, the leading provider of high-tech out-of-hospital healthcare, to bring healthcare services across the trust closer to patients’ homes with a series of pilot projects.
The following projects aim to develop personalised care for patients in their own homes, shortening the time they have to spend in hospital and preventing them from being admitted unnecessarily:
- Enhanced Supported Discharge following fractures
- Acute Community-based Care Service for long term conditions such as COPD
- The Family Liaison Service
- The Herceptin at Home service
- Access and Wellbeing

Among the groundbreaking new services being offered are a rapid response team, new care pathways and a dedicated care bureau. The care bureau will provide a 24 hour telephone nurse triage facility, enabling patients with these services to call in when they need to receive medical advice.
